Gift Card

All participants in A Dose of Knowledge: Safely Managing Your Pain webinars who complete both the pre- and post-webinar surveys, will be entered for a chance to win a gift card. To be entered you must provide your contact information, including a valid e-mail address. Every 3 months for a year, 10 winners will be chosen from the entries to receive an Amazon $25 gift card. Winners will be notified by e-mail and will need to accept their prize within 30 days. Accepting requires providing a valid mailing address for the gift card to be mailed.
