No matter your health status, a health organizer is a simple yet very helpful tool to keep track of important health information, so you don’t have to search high and low every time you or your health care provider needs to know information related to your health.
Your health organizer contents will be slightly different compared to someone else because it’s based on what you find to be the most important to you. Some general things you could keep track of are medications (how often you take them, strength of your prescription, etc), vaccinations, questions you have for your doctor, your personal and family medical history, recent health changes you may have experienced, doctor recommendations and a nutrition and fitness log. It may initially take a little time to gather all of this information, but once you make a habit of recording key events every time your health changes, it will help you remember important dates such as the last time you received your flu vaccine, when you need to call your doctor for a prescription refill, and your next annual check-up appointment. Initial time invested in organizing could save tons of time searching for this information later. The Alliance for Aging Research has a personal health organizer available for those who have been diagnosed, or are taking care of someone, with Alzheimer’s disease. Some of the worksheets that are available can be used by anyone who wishes to keep a health record at home and are not specific to Alzheimer’s disease. Take a few minutes to glance over the included worksheets and decide if they would be helpful for you. Don’t hesitate to create your own!
